Connections Health Solutions is a physician-owned, multifaceted behavioral health company whose mission is to address any behavioral health need at any time. We are hiring Crisis Workers at the Crisis Response Center (CRC) whose primary focus is to provide crisis social work services to individuals and families on an outpatient basis including counseling and crisis intervention, short-term case management, and information and referrals to community resources. In addition, the Crisis Worker will assist Providers by acting as a consultant for individuals with complex psychosocial needs.

  • Promotes a welcoming environment and excellent customer service with individuals and families served by the program
  • Conducts the initial triage and assessment of individuals and determines their level of acuity during the intake process.
  • Works collaboratively with the Crisis Clinical Team (Medical Director, Physicians, RNs, BHTs, Recovery Support Specialist, Family Support Specialist, and Youth Support Specialist) to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individual's needs
  • Works with families and caregivers, helping to educate them and the individual about wellness and the principles of recovery
  • Performs a psychosocial evaluation of the person's needs and strengths, including the individual's physical, social and emotional functioning as well as an assessment of financial resources and family and community supports
  • Develops an individualized and person-centered treatment plan which includes arrangements for a referral to services or natural supports as appropriate
  • Uses motivational interventions to assist individuals in meeting their goals and moving along their paths to recovery
  • Provides community resource information and performs referral and liaison activities as necessary to ensure the person's success in returning to the community
  • Provides case management services to individuals as appropriate

  • Education: A master's degree in a behavioral health field
  • At least two years of experience as a BHT or BHP in a "behavioral health service", which means the assessment, diagnosis, or treatment of an individual's behavioral health issue. Assessment experience must include assessing for risk, acuity and lethality
  • Must have experience performing mental status exams and have a working knowledge of the DSM V
  • Must have demonstrated skills in triage, assessment and discharge planning for an individual receiving behavioral health services
  • Behavioral Health Professional (BHP) experience with an Arizona behavioral health license or independent license is preferred
  • Knowledge of the principles of recovery
  • Knowledge of Arizona behavioral health system principles
  • Must have prior experience in performing triage and assessment
  • Possess objectivity and good judgment relating to difficult and emotionally-charged situations
  • Exceptional interpersonal skills, a collaborative style and the ability to communicate effectively at all levels
  • Healthcare system experience; preferably multi-entity
  • Proficient in Microsoft Office and electronic health record software
