Independent Living Specialist II

The Independent Living Specialist II is responsible for assisting youth in the transition from foster care to an independent, stable living environment. In addition, position will assist youth in gaining and sustaining employment, accessing person and community resources, while providing training and support, based on contracted service.   

 

 

Essential Job Functions:  

 
     
  •  Maintains contact with youth, collateral persons and agencies to assist youth to develop and maintain life skills and accomplish established goals. 
  •  
  •  Develops and reviews individualized goals and service plans. 
  •  
  •  Provide transportation for youth to group meetings, education outings and to employment settings. 
  •  
  •  Provides individual support and services focusing on life skills development. 
  •  
  •  Provide group workshops focusing on issues common to adolescents preparing for independent living. This includes exploration and skill building in self-esteem, goal setting, healthy relations, broad vocational and career issues, safety, and emotional preparation for adulthood. 
  •  
  •  Assist youth to develop and maintain a contract with the Department of Child Safety when eligible. 
  •  
  •  Assists youth in developing and accessing scholarships and grants for ongoing education and training. 
  •  
  •  Completes daily case notes, monthly reports and billing, and state-mandated monthly, quarterly and annual data reports. * Participates in team meetings with supervisor and other staff and agencies. 
  •  
  •  Safely and effectively monitor consumer activities, with or without a reasonable accommodation. 
  •  
  •  Performs other duties as assigned which are consistent with the general responsibilities of the position.

Minimum Qualifications and Experience:  

 
     
  •  Must be 21 years of age (licensing requirement). 
  •  
  •  Must be able to provide DPS fingerprint clearance. 
  •  
  •  Minimum of 1 year experience working with adolescents or young adults.  
  •  
  •  Bachelor’s Degree in social service, or related field is required.
  •
