The Supportive Housing Specialist is an integral part of a multi-disciplinary team of mental health clinicians. The Supportive Housing Specialist provides 80% of housing based case management in consumer homes, shelters, and other community locations. The Supportive Housing Specialist provides direct housing support services to assist to obtain and maintain community-based housing security and stability. Coordinates, monitors, and provides direct services in a community setting for members. May participate in after-hours care, on-call coverage, and support to all recipients being served on the team.
- Bachelors’ degree in Social Work, Psychology, or related field, or equivalent experience and education
- At least two (2) years of experience working in the medical field and understanding chronic medical conditions.
- At least one (1) year of experience working in the public behavioral health system (preferably with members with SMI)
- At least one (1) year experience providing housing-related training or support
- Strong Familiarity with the Arizona Landlord Tenant Act, Fair Housing, Housing First, and other housing regulations and programs
- Understanding of culturally and ethnically diverse members and families
- Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery
- Advanced knowledge of public system and community resources
- Understanding of and knowledge as related to harm reduction, 12-step and other recovery support models
- Strong knowledge of wellness, recovery, self-help, and client-driven support plans
- Effective communication and written skills are essential
- Proficiency with computer and databases
- Bilingual skills in Spanish preferred
- Must possess a valid Arizona driver’s license, insurance coverage, reliable transportation, and a clean driving record
- Must pass a pre-employment drug screening
- Must be able to obtain full fingerprint certification
