Care Coordinator (Case Manager) - Desert Rose

Job Description


Community Partners is a growing Integrated Care Provider, looking for passionate and qualified professionals to fill our Care Coordinator team.  We offer benefits and competitive wages.



The Care Coordinator is responsible for attending to Integrated Care members for behavioral interventions for a variety of member issues including depression, anxiety, pain management, and other chronic medical conditions for which the member would benefit from behavior changes, coaching, or additional support. The Care Coordinator may also intervene for crisis response, or to support the member and care teams in establishing behavioral norms for the continued care relationship.

The Care Coordinator provides psycho-education and case management follow-up and referral to improve member health outcomes. The Care Coordinator directly supports the Behavioral Health providers including Behavioral Health clinicians as well as the Psychiatric Nurse Practitioners, conducting case management around medication monitoring, behavioral activation, maintenance, and linking members with resources in the community.

The Care Coordinator contributes equally to the member care teams through huddles, case consultation, member advocacy, mental health education, and collaborative team meetings with medical staff. The Care Coordinator is expected to be professional, punctual, maintain regular attendance, cooperative, organized, and enthusiastic at all times.

Required Qualifications (Knowledge, Skills, Abilities, Education, Training, and Licensure)

  • High School Diploma or GED equivalent required; Bachelor’s degree preferred
  • Experience or education in related fields such as social work, psychology, or counseling chemical dependency, or Registered or Certified AOD Counselor, or other related human service field
  • Experience working in medical settings preferred but not required
  • Experience in a variety of assessments including substance abuse preferred OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job
  • Bi-lingual ability preferred but not required
  • Knowledge and skill working with diverse populations
  • Strong verbal and written communication skills
  • Strong interpersonal skills
  • Ability to abide by standards of professional ethics
  • Compliance with all state and federal laws and regulations as they pertain to the position including HIPAA, sexual harassment, Mandated Reporting, Scope of Practice, OSHA,
  • Ability to understand and promote the mission, vision, and values of Community Partners Integrated Healthcare both in the work place and in the community
  • Computer literacy including word processing and e-mail
  • Valid Arizona Driver’s License, with an insurable driving record
